Name Change
Has anyone here filed a petition for name change so that your last name matches your partner? I am filling out the form, and I am struggling with the answer to "reason for request". Any help?

need info
i actually would like to look into that for myself. now that we are trying to have another baby the new baby will have my partners last name. same as our first child. so i would like to change mine as well. PLEASE i dont even know where i have to go to begin the process....and information you have could you please let me know. as for the reason for the change could you put so you share the same family name as your partner or soon to be child? or is that not a good enough reason?? please let me know

I changed mine here in IL. It was EASY!! Put the ad in the paper. 6 weeks later, i went to court and it was done!
Go to you county's website and call the County Clerks office and ask what you need for a legal name change. I remember saying I was changing my name to match that of my family. I wasn't asked any questions at all.
